CLINIC TEN SHIBUYA — Handling of Personal Information

CLINIC TEN SHIBUYA (hereinafter, "the Clinic"), in providing medical services (including the provision of medical information; hereinafter, "the Service"), respects the importance of the personal information, special-care-required personal information, and other information relating to users of the Service (hereinafter collectively, "Personal Information, etc."). To ensure its thorough protection, the Clinic complies with the Act on the Protection of Personal Information and other related laws and the guidelines published by the competent authorities (hereinafter, "Personal Information Protection Laws"), and establishes this Privacy Policy to appropriately protect Personal Information, etc.

1. Personal Information and Special-Care-Required Personal Information

• "Personal Information" means information about an individual that can identify a specific individual by name, date of birth, address, identification number, code, image, voice, or other description contained therein (including information that, even if it alone cannot identify an individual, can be easily cross-referenced with other information to identify a specific individual), as well as information regarding the Individual Number Card or Notification Card, and information subject to regulation or protection as "personal information or specific personal information" under applicable laws.
• "Special-care-required personal information" means personal information that includes descriptions requiring special care in handling so that the individual does not suffer unfair discrimination, prejudice, or other disadvantage — including medical history recorded in medical records, treatment/dispensing information that the Clinic learns about a user's physical condition, illness, or treatment, results of health checkups and health guidance, the fact of a disability, and the fact of having suffered harm from a crime.

2. Basic Policy on Management of Personal Information

• The Clinic collects, uses, and provides Personal Information, etc. appropriately.
• The Clinic takes measures to prevent unauthorized access to, loss of, and leakage of Personal Information, etc.
• The Clinic complies with the Personal Information Protection Laws, norms, and internal rules regarding Personal Information, etc.
• The Clinic continuously strives to improve the protection of Personal Information, etc.
• If a problem arises in the protection of Personal Information, etc., the Clinic will report to the supervisory authority and promptly implement corrective measures.

3. Purposes of Use

The Clinic will use the collected Personal Information, etc. for the following purposes.

Purposes necessary for providing medical care to users

• For internal use within the Clinic: medical services provided to users; health insurance administration; among the Clinic's management operations relating to users — accounting/bookkeeping, reporting of medical accidents, and improvement of the user's medical services; provision of information to users about the Clinic's medical services and medical-related products by e-mail, SNS, or postal mail.
• Involving provision of information to other businesses: coordination with other hospitals, clinics, midwifery centers, pharmacies, home-visit nursing stations, and long-term-care service providers; responses to inquiries from other medical institutions; seeking opinions/advice from external physicians for a user's treatment; outsourcing of specimen testing and other operations; explanation of conditions to family members; among health insurance administration — outsourcing of insurance clerical work, submission of receipts to review/payment organizations and responses to their inquiries; notification of results to businesses that commissioned health checkups; and consultation or notification to professional medical bodies and insurers regarding physician liability insurance, etc.

Other purposes of use

• For internal use: basic materials for maintaining/improving medical services and operations; cooperation with student training conducted within the medical institution; case studies conducted within the medical institution.
• Involving provision of information to other businesses: provision of information to external auditing bodies as part of management operations.

4. Provision of Personal Information

The Clinic may provide Personal Information, etc. to a third party in the following cases:

• When disclosure is requested by a court, public prosecutor's office, police, tax office, bar association, or an authority with equivalent powers;
• When disclosing to financial institutions, credit card companies, collection agents, or other payment/settlement providers in order to settle medical fees and other amounts payable to the Clinic;
• When outsourcing all or part of the Clinic's operations to a third party;
• When disclosing to a party that owes a confidentiality obligation to the Clinic;
• When necessary for the Clinic to exercise its rights;
• When disclosing to a successor of the business in the event of a merger, business transfer, or other succession;
• When permitted by the Personal Information Protection Laws or other laws (including the Medical Care Act, the Long-Term Care Insurance Act, and the Act on the Prevention of Child Abuse).

5. Cookies, etc.

The Clinic may use technologies such as cookies, DoubleClick cookies, or web beacons to obtain log files of users' service-usage information (such as the model name of the user's device, OS version, language/region/time-zone settings, IDFA (advertising ID on iOS), IP address, access date and time, browsing information, and behavior history — excluding, however, information that can identify a specific individual).

6. Changes to the Privacy Policy

The Clinic may change this Privacy Policy from time to time. A change takes effect when the revised Privacy Policy is posted on the Clinic's website or otherwise made available for viewing. If you use the Service after a change, you are deemed to have accepted and agreed to the revised Privacy Policy.

7. Disclosure to the Individual, etc.

• If a user requests disclosure, correction, addition, or deletion of their personal information, the Clinic will investigate without delay and respond appropriately and promptly.
